Waiver:

I represent that I am the parent or legal guardian of the participant(s) named or I have obtained permission from the parent/legal guardian of the participant(s) named to execute this agreement on their behalf.

 

I, for myself and the participant(s) named, acknowledge and understand that there are known and unknown risks associated with all exercises and use of all equipment, machinery or facilities, participation in activities, classes, programs, personal trainings or instructions and all the aforementioned shall be undertaken voluntarily by me, at my sole risk with knowledge of the dangers involved.

 

I, for myself and the participant(s) named, agree that the Princeton Arts & Sports Academy/Patrick School Basketball Training Academy/Windsor Athletic Club its owners, officers, servants, agents or employees shall not be liable for any costs, claims, demands, injuries, deaths, damages, liabilities, actions or causes of actions whatsoever, to person or property, arising out of or connected with the use of any of the services, equipment, machinery or facilities or participation in any activities, classes, programs, personal trainings or instructions of the health club or the premises where the same are located including those arising from (i) acts of active or passive negligence on the part of the Princeton Arts & Sports Academy/Patrick School Basketball Training Academy/Windsor Athletic Club, its owners, officers, servants, coaches, agents, or employees, (ii) the sudden and unforeseen malfunctioning of any equipment or machinery and (iii) participants slipping and/or falling while in the health club or on the premises, including adjacent sidewalks and parking areas, and do hereby expressly forever waive, release and discharge the health club, its owners, officers, servants, agents or employees from any and all such costs, claims, demands, injuries, deaths, damages, liabilities, actions, or causes of action.

 

I, for myself and the participant(s) named, am of physical ability to participate and legally competent to understand and complete this agreement. I hereby execute this agreement without coercion.

 

I, for myself and the participant(s) named, understand that entry by myself and the participants named constitutes consent for Princeton Arts & Sports Academy/Patrick School Basketball Academy/Windsor Athletic Club to use any film, video, or likeness of participants for any purpose what so ever, without payment to the participant.

 

In case of emergency, I give permission to the employees of the Princeton Arts & Sports Academy/Patrick School Basketball Training Academy/Windsor Athletic Club to evaluate, respond to, and notify the emergency medical services if needed for the participant(s) listed below, until such time as a parent/guardian can be notified.

Cancellations, Credit & Transfer Policy: 
If you need to change or cancel your child’s enrollment, we offer several options based on the following guidelines:

  • May 15th, 2019 (or earlier). Partial cancellations on or prior to this date are permitted ($50 administrative fee, per child). Full cancellations made on or prior to this date are subject to a $100 fee, per child. There are no refunds after May 15th, 2019.

  • May 16th, 2019 through June 1st, 2019: We will issue you a credit only for program fees, to be used for alternate weeks of the summer institute that are applicable to your child. Please Note: No credits will be given out after June 1st, 2019 regardless of your registration date, vacation, or illness.

  • May 16th, 2019 through June 1st, 2019: Changes in enrolled weeks that do not involve a reduction in weeks are still permitted with no penalty and based on availability. Please Note: No refunds or credits will be given out after June 1st, 2019 regardless of your registration date, vacation, or illness.

  • June 1, 2019 (or later): You may add weeks at any time without a late fee or charge.  Switches or changes in enrollment that do not involve a reduction in weeks are also permitted, but are subject to a $25.00 transfer fee per child for each request.

Guiding Principles:

  • To ensure that the rights of all individuals are protected while attending the TPS Basketball Training Institute.

  • To establish the safest and best possible learning and development environment for all academy participants.

  • To ensure that breaches of the rules and code of conduct are treated in a fair and consistent manner.

Expectations

  • All parents and institute participants have the responsibility to treat one another, staff and property with respect.

  • All parents and academy participants have the responsibility to act and behave in a way which does not endanger, intimidate or interfere with the participation of others.

  • Institute Participants have the responsibility to follow the instructions given by Summer Institute Staff.

  • All parents and institute participants have the responsibility to behave according to this code of conduct.

 

Rules – the following forms of behavior are considered a violation, are unacceptable and could result

  • in the immediate dismissal of camper(s):

  • Cigarettes or any tobacco products, alcohol, non-prescribed drugs, weapons and pornographic material MUST NOT be brought to camp.

  • Using language which is offensive, sexist or racist.

  • Fighting, bullying, social bullying or any other forms of aggressive behavior.

  • Leaving camp boundaries without permission.

  • Behaving in a manner which is potentially dangerous to self and others.

  • Behaving in a manner which damages or vandalizes the property of others or the Princeton Arts & Sports campus.

  • Cell Phone use is strictly prohibited during summer institute hours.

 

Regulations – the following regulations must be adhered to at all times:

  • Medication is not permitted at the Summer Basketball Institute, without prior written consent by Martin Whitfield of the Princeton Arts & Sports Academy. Please contact Martin Whitfield of the Princeton Arts & Sports Academy if your child requires special medication during Institute hours.

  • Photos & Video are not permitted without permission.

  • Any breach of the Rules or the Code of Conduct will initiate a disciplinary action. The Princeton Arts & Sports/Patrick School Basketball Training Academy reserves the right to suspend or dismiss a camper’s participation in program activities, without refund of program tuition, if such disciplinary action is required. Before such a decision is fully made, the camper and parent/guardian will meet with an official Princeton Arts & Sports/Patrick School Basketball Training Academy Staff & Representative to determine the best course of action.
